Most rotating equipment failures that get blamed on pump & mechanical seal maintenance start as a lubrication or alignment miss. The housing is the last thing to come off, not the first. Check lubrication method and interval against the actual duty. Check alignment with the coupling still telling the truth. If the numbers do not hold, the insert will not save you.
Cavitation, a dry-run, and a flush plan that was never piped will kill a seal faster than a cheap face. Confirm suction, alignment, and the actual API plan before you order another cartridge.
