Most rotating equipment failures that get blamed on centralized & automated lubrication systems start as a lubrication or alignment miss. The housing is the last thing to come off, not the first. Check lubrication method and interval against the actual duty. Check alignment with the coupling still telling the truth. If the numbers do not hold, the insert will not save you.

Most “bearing failures” on the plant floor are lubrication or alignment. The procedure millwrights actually run happens before anyone pulls a housing: what the bearing is telling you, grease that matches the duty, and an alignment you can defend.
